Our assessment results are unique and reflect your specific needs. We provide this service because we are passionate about helping seniors age successfully and supporting the family members who care for them.  

No matter what you decide, this Care Consultation can give valuable information as you make decisions.

24/7 Care Available

•  We will conduct a personal needs assessment.
• We will ask questions about common activities of daily living to understand your needs.
• Once the Care Manager has assessed the situation, he or she will explore care options the family may benefit from, based on any gaps or deficiencies noted.
• We will conduct a Home Safety Check to help identify any environmental safety concerns, such as trip hazards, lack of grab bars in wet areas, misuse of extension cords or space heaters – or any other safety concerns.
• The Care Manager conduct a social assessment to identify any physical or emotional risks associated with isolation and loneliness.
• We will conduct a Care Coordination Plan to  discuss any health concerns, medication management and other issues that should be addressed.
• Finally, the Care Manager will review the results with you and make recommendations. Together, you can discuss the hours and types of care that would best support your family.
